Your First Visit

At Levas Physical & Occupational Therapy, Our Strength is Bringing Yours Back! You have just taken the first step towards rehabilitating your injury/condition.

In order to initiate your treatment first you will need to do the following:

STEP 1- Getting you scheduled for an appointment is our first priority. You will need to call our scheduling department at 858-455-1222 to arrange your initial evaluation. Upon your initial call, you will be asked questions like: Did you have a recent surgery? How soon do you need to come in? Which of our six locations are you interested in: UTC/La Jolla, Poway, Mission Valley (2 locations), El Cajon or the Bonita/Eastlake area? Who is your referring physician? Was this an auto or work related injury? Will we be billing your insurance? If so, we will obtain that information from you over the phone.

Unless you have a “self referral” insurance plan, we will need a referral/prescription from your physician. To initiate things quickly, this can be faxed to Elvy’s attention at our administrative offices at 858-455-7101. We will also need for you to bring the original with you when you come in.

STEP: 2- We ask that you arrive 15 minutes early to fill out your paperwork. If you wish to avoid this then feel free to print the new patient information documents here on our web page, complete them at home or work and arrive only five minutes early so we can put your chart together and your therapist can begin your evaluation.

IMPORTANT INFORMATION- If you are using your private insurance (i.e., HMO, PPO or POS plan, you will be given your benefit quotation from your insurance company). We ask that you review your benefits carefully, by checking your benefit booklet, and let us know if there are any discrepancies. Remember you know your policy better than we do. We can only rely on what your insurance company tells us, and occasionally they are wrong. If you have any questions about your benefits or our fee’s you can ask to speak with Kathleen, our Administrator. If she is not available, ask to leave a message for her to contact you, she will be happy to explain anything you need help with.

Your initial visit will be up to an hour and a half, after that your visits will be about an hour. If your injury/condition is the low-back region or lower, you will need to bring a pair of shorts with you each visit. You can either wear them in or use our changing room upon your arrival. If you forget them, we do have a few extra pairs you can borrow a pair from us.
